About the Role:

This role is an exciting opportunity to join our team as a Rehabilitation Support Worker. You will be working at Osman House, a 16-bed unit in Scholes, Leeds, which provides rehabilitation to people with an acquired brain injury. Your main purpose will be to provide personalised support to the people we support, following individual support plans to meet their needs and aspirations. You will help develop independent living skills, such as personal hygiene, meal preparation, exercise, leisure activities, socialising, and personal care. The focus is on helping individuals regain the skills they have lost due to their brain injury. You will be a key member of the team, contributing to the team ethos and supporting service users and colleagues appropriately.

Main Responsibilities:
  • Facilitate the daily routines of the service users, promoting a high standard of personal hygiene at all times.
  • Participate in the development of service user support plans, risk assessments, and programmes, complete written records as necessary, and in accordance with the guidelines provided.
  • Implement individual support plans to meet the needs of the service users, highlighting any issues or concerns to the Team Leader to ensure quality services are provided at all times.

You will enjoy teamwork and take pride in getting a job done well alongside your colleagues. Excellent face-to-face communication skills and a good standard of literacy are essential. You will also have good time management skills and the ability to prioritise your own workload.
