  • Collaborate with parents, carers, and professionals to gather information about a child's SEND
  • Conduct assessments and reviews to determine the child's needs
  • Facilitate multi-disciplinary meetings to develop EHCPs
  • Act as the central contact for all EHCP-related communications
  • Coordinate assessments, reviews, and meetings for timely information exchange
  • Maintain accurate records and documentation
  • Liaise with educational professionals to implement EHCP provisions
  • Monitor and review the child's progress, ensuring EHCP provisions are met
  • Participate in annual reviews and evaluate EHCP effectiveness
  • Provide reports and updates to Local Authorities and relevant bodies
