Job Summary
We are seeking a highly skilled and compassionate Care Facilitator to join our team. As a Care Facilitator, you will play a vital role in providing top-notch service to our patients, supporting their ongoing care, and ensuring that their records are maintained and up-to-date.
Main Responsibilities
- Act as the link between clinicians and clinical correspondence in the practice, scanning, coding, and matching externally received documentation at point of source using SystmOne.
- Provide and maintain a high standard of patient communication via telephone, letter, and SMS, delivered with a customer focus to the practice population.
- Work alongside clinical and administrative colleagues to facilitate the provision of preventative healthcare to the practice population.
- Liaise with patients and care facilitate enquiries so as to deliver healthcare outcomes in line with practice objectives and contractual responsibilities.
- Provide secretarial support and cover.
Requirements
- GCSE grade A to C in English and Maths.
- Experience working with members of the public.
- Experience with using IT.
- Stellar communication and interpersonal skills.
- Team player with a dash of independence.
