Care Coordinator

We are seeking a highly skilled and compassionate Care Co-ordinator to join our team at C M Community Care Limited. As a Care Co-ordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.
Key Responsibilities- Assist with the creation of rotas and working patterns for all staff to ensure seamless service delivery.
- Coordinate daily service delivery, managing inquiries from staff, clients, and other healthcare professionals.
- Conduct formal checks on staff in the field and undertake initial care assessments.
- Develop and update client care plans to ensure the best possible outcomes.
- Contribute to delivering a high standard of care through effective communication and teamwork.
- Provide cover for care delivery in contingency situations.
- A full UK driving license and car are essential.
- A smartphone is required for communication and data management.
- We seek applicants with a natural caring approach and excellent communication and teamwork skills.
- Experience in a care setting is preferred, but not essential as we provide comprehensive training.
- A passion for caring for others and a commitment to delivering exceptional care services are essential.
