Job Description:

We are seeking a highly skilled and experienced Care Coordinator to join our team at Invest Solutions Ltd. As a Care Coordinator, you will play a crucial role in ensuring the smooth operation of our care services, working closely with our care teams to deliver high-quality care to our clients.

Key Responsibilities:

  • Service Development: Develop and implement effective care plans that meet the unique needs of our clients, ensuring their dignity and respect are maintained at all times.
  • Operational Excellence: Manage and coordinate the care worker rota, ensuring that clients receive the best possible care while maintaining a high level of staff satisfaction.
  • Client Support Specialist: Provide exceptional support to our clients, their families, and our care teams, ensuring that all needs are met and exceeded.

Requirements:

  • Education and Experience: NVQ/QCF level 3 in Health and Social Care, with at least 2 years' experience working as a care coordinator for a Domiciliary Care Agency.
  • Skills and Qualifications: Excellent IT skills, knowledge of CQC Compliance, and experience in conducting care assessments, care reviews, and motivating care staff.
  • Personal Qualities: Organized, methodical, and a good eye for detail, with the ability to work flexibly and as part of a team.
