We cater to anyone in our community confronting a life-limiting illness. By utilizing our extensive range of services, we craft personalized care pathways that commence at diagnosis and extend through bereavement support for families, caregivers, and children.
COMMUNITY STAFF NURSE | Band 5 £28,407 - £34,581 per annum
This role is classified as Band 5, with opportunities for advancement to Band 6 as positions become available.
The ideal candidate will collaborate with our dedicated Community Palliative Care Team, contributing to the provision of palliative care through expertise in decision-making, advance care planning, and holistic symptom management.
Key Responsibilities
- Functioning flexibly as part of a supportive team within the Dorothy House Community Team.
- Conducting comprehensive assessments of patients and families referred to the team, whether in their homes, in a clinical setting, or virtually.
- Delivering nursing care that is centered on the patient and family, recognizing individual needs and ensuring high standards of care.
- Developing skills in other community-related departments within Dorothy House Hospice Care that enhance the patient and family experience.
- Assessing the bereavement needs of families and facilitating referrals for follow-up with the Bereavement Service.
- Utilizing learning opportunities to enhance the knowledge and skills of health and social care colleagues.
- Providing mentorship to learners pursuing relevant health and social care courses.
Working Hours
We offer a variety of full-time and part-time positions across different locations.

Candidate Profile
Essential Qualifications:
- Registered nurse with a relevant qualification.
- Clinical experience at Band 5 or equivalent.
- Team-oriented with excellent communication abilities.
- Demonstrated commitment to ongoing professional development.
- Valid UK driving license with access to a vehicle.
Desirable Qualifications:
- Clinical experience in a relevant specialty.
- Knowledge or experience in palliative care.
- Strong clinical assessment and analytical skills.
- Experience in teaching or training.
- Research capabilities.
- Community experience or qualification.
