Key Responsibilities- Care for patients with a life-limiting illness, likely to be in the last 12 months of life
- Support symptom control management for patients attending with physical, psychological, and emotional symptoms
- Care for patients in the last days/weeks of life
- Care for patients admitted for symptom management
- Be part of a wider Specialist Palliative Care Multidisciplinary Team
- Deliver fundamentals of care based on patient wishes
- Support communication with carers
- Link with teams across acute, community, and primary care
- Clinically lead the team on a day-to-day capacity and in the absence of the Band 7 lead nurse
- Complete quality audits
