Job Summary:The Phoenix Health Group is seeking a skilled and compassionate Patient Care Adviser to join their team. As a key member of the Practice team, you will play a vital role in providing exceptional patient care and administrative support.

Key Responsibilities:
  1. Assist patients in accessing the appropriate healthcare professional or service in a courteous and efficient manner.
  2. Undertake various administrative duties to ensure the smooth operation of the Practice, including clerical support to clinical staff and other team members.
  3. Facilitate effective communication between patients, healthcare professionals, and other agencies.
  4. Process telephone requests for appointments, visits, and consultations, and direct callers to the appropriate healthcare professional.
  5. Take messages, pass on information, and maintain accurate records.
  6. Process repeat prescriptions in accordance with Practice guidelines.
  7. Enter data into the Practice's computer system, allocate and collate information, and record data in accordance with Practice procedures.
  8. Initiate contact with patients, team members, and healthcare agencies, and respond to requests and queries.
  9. Clear and restock consulting rooms as required, and provide refreshments for staff and visitors.
  10. Maintain the reception area, notice-boards, and leaflet dispensers in a tidy and organized manner.
  11. Be flexible to work additional hours to cover colleagues, as required.
Confidentiality:The post-holder will have access to confidential information relating to patients, carers, Practice staff, and other healthcare workers. All such information is to be regarded as strictly confidential.

Health & Safety:The post-holder will assist in promoting and maintaining their own and others' health, safety, and security as defined in the Practice Health & Safety Policy, including using personal security systems, identifying risks, and making effective use of training.

Equality and Diversity:The post-holder will support the equality, diversity, and rights of patients, carers, and colleagues, including respecting patients' privacy, dignity, needs, and beliefs, and behaving in a welcoming and inclusive manner.

Personal/Professional Development:The post-holder will participate in any training program implemented by the Practice, including an annual individual performance review, taking responsibility for own development, and contributing to the effectiveness of the team.

Quality:The post-holder will strive to maintain quality within the Practice by alerting other team members to issues of quality and risk, assessing own performance, and contributing to the effectiveness of the team.

Communication:The post-holder will recognize the importance of effective communication within the team and strive to communicate effectively with other team members, patients, and carers, and recognize people's needs for alternative methods of communication.

Contribution to the Implementation of Services:The post-holder will apply Practice policies, standards, and guidance, discuss with other members of the team how the policies will affect their work, and participate in audit where appropriate.
