**Requirements:**
* Previous experience in care coordination or a related field
* Valid driver's license and access to a reliable vehicle
**Schedule:**
* Monday to Friday, with occasional weekends and on-call duties
**Salary:**
* Competitive salary range of £27,000 - £36,000, based on experience
* Paid mileage for work-related travel
