Job Overview

We are seeking an experienced Care Coordinator to join our dedicated team in St Helens, Merseyside. As a skilled Care Coordinator, you will play a key role in the continuous delivery of care to service users.

Your primary responsibility will be to manage a caseload of care packages, understanding the varied nature and fast-pace of the role, along with all elements of associated care delivery.

You will apply your gained communication and relationship building skills to effectively manage rota's for your care teams, respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.

No matter which care setting you've obtained your Care Coordination experience in, you'll be aware that no two days are the same, so you'll already be able to evidence your adaptability and aptitude to work well under pressure, using your initiative and thinking on your feet.

Key Responsibilities
  • Manage a caseload of care packages
  • Effectively manage rota's for care teams
  • Act as the first point of contact for service users and their families
  • Respond efficiently to day-to-day changes
Requirements
  • Driving Licence and access to a vehicle
  • Commitment to supporting the out of hours on call service
  • Competitive salary between £29,460 - £35,460 per annum
