Pediatric Palliative Care Nurse

Position Overview
The successful candidate will be an integral part of a dedicated care team, providing community-based palliative nursing services. This role involves collaborating with families who are caring for children and adolescents facing life-threatening and life-limiting conditions.
The nurse will deliver comprehensive holistic nursing care to children and young individuals with intricate medical requirements, whether in their homes or in other settings where respite care is requested, including hospital environments. Emotional support and guidance will be provided to the children, young individuals, and their parent/carers on the caseload. The nurse will also be responsible for the planning, execution, and assessment of personalized care during each respite visit in the absence of the parents or guardians. Additionally, collaboration with the end-of-life care team leader will be essential to ensure effective end-of-life care for children within the caseload.
Main Responsibilities
As a member of the care team, your primary responsibilities will include:
- Delivering high-quality, individualized holistic care and support to children and young individuals with life-limiting conditions and their families, fostering empowerment, advocacy, and collaboration.
- Supervising the child or young individual in the absence of their parents or guardians.
- Facilitating effective communication with all members of the care team.
- Promoting evidence-based practices that enhance the care of children within the caseload, adhering to clinical governance standards.
- Executing all necessary nursing tasks within the home setting, developing and implementing additional clinical skills as required while maintaining competency.
- Managing your workload effectively during respite visits and office-based responsibilities.
- Understanding and adhering to organizational policies and guidelines.
- Encouraging effective communication with colleagues and partner organizations.
- Collaborating with your supervisor to enhance services and improve the quality of care for children and young individuals.
- Ensuring compliance with health and safety regulations and reporting any incidents to your supervisor.
