We are seeking a highly skilled and experienced Team Lead to join our Community Access Team at Norfolk Community Health and Care NHS Trust. As a key member of our team, you will be responsible for providing operational leadership and line management to the Community Access Team, ensuring a seamless and integrated service to our service users.
Key Responsibilities- Operational Line Management: Lead and manage the day-to-day operations of the Community Access Team, ensuring a 7-day, 8am-8pm service is provided.
- Service Development: Facilitate the discharge of patients to community hospitals and procured beds, working within statutory timeframes and operational targets.
- Collaboration and Partnership: Work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
- Team Management: Manage the Community Access Team caseload, ensuring effective use of staff time and resources.
- Policy Development: Propose and implement departmental policy changes for the management of patients referred to the service, networking with other specialist practitioners.
- Representation: Represent the service and NCH&C at strategic meetings with partner organisations as requested.
We are looking for a candidate with:
- Essential Qualifications: Registered Nurse/Therapist, First-level degree, diploma or equivalent, Current registration to a professional body, Recognised management qualification or evidence of working at that level.
- Desirable Qualifications: Masters-level degree.
- Essential Experience: Experience of leading a team, Experience of working in a multi-agency environment, Experience of managing budgets, A track record of successful service and policy development and implementation, Performance Management Experience, Evidence of post-graduate development.
- Desirable Experience: Project management, Experience of managing a multidisciplinary team.
- Essential Skills and Abilities: Change management, Knowledge of additional specialist areas through in-house training and short courses, Able to provide high standards of care, Able to manage stressful situations, Able to concentrate in busy, pressured environments and prioritise time accordingly, Work flexibly to accommodate patient and service needs, Be aware of own limitations and when to seek support, Concentrate when undertaking patient care and inputting data/patient records, Working knowledge of current issues within clinical practice and service development, Good observational and reporting skills, Analytical skills with the ability to problem-solve complex situations.
