About the Role
We are seeking a highly skilled Registered Nurse to join our Health Visiting service across Hampshire. As a Community Health Nurse, you will be responsible for delivering delegated commissioned elements of the Healthy Child Programme as a member of the 0-19 public health nursing service.
Main Responsibilities
- Identify health needs with children and families and plan, deliver, and review appropriate and evidence-based interventions to improve outcomes.
- Develop close working relationships with our Barnardo's workers to support the development of preventative community-based approaches and personalized care.
- Work collaboratively with the Health Visitor to deliver high-quality care and services to children and families.
