Job Overview

We are seeking a highly skilled and compassionate Registered Mental Health Nurse (RMN) to join our Memory Clinic team. As a key member of our multidisciplinary team, you will provide specialist nursing care and support to individuals with memory concerns and their families.

Main Responsibilities
  • Provide comprehensive nursing assessments and interventions for individuals with memory concerns
  • Develop and implement individualized care plans to promote optimal health and well-being
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Provide education and support to individuals, families, and caregivers
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements
  • Professional qualification as an RMN or OT
  • Experience working with older adults in a mental health setting
  • Excellent communication and interpersonal skills
