Position Overview

The Clinical Nurse Specialist will play a pivotal role in delivering nursing services within a multidisciplinary healthcare team, ensuring high-quality care for the designated patient population. This role encompasses various clinical domains, including health promotion, chronic illness management, and preventive health services, while also contributing to the development and review of clinical policies and procedures.

Key Responsibilities

The following are the primary responsibilities associated with this position. Additional tasks may be assigned based on operational needs:

  • Design, implement, and integrate health promotion and wellness initiatives.
  • Develop and assess individualized treatment plans for patients with chronic conditions.
  • Identify and support patients at risk of developing long-term health issues, aiming to prevent negative health outcomes.
  • Deliver routine nursing care in accordance with clinical guidelines and evidence-based practices.
  • Administer wound care and related services to patients.
  • Provide travel health consultations and vaccinations.
  • Request and manage laboratory services as needed.
  • Process and interpret laboratory results for patient care.
  • Conduct chronic disease management clinics, coordinating care and referrals to specialists as necessary.
  • Maintain precise clinical documentation in compliance with relevant legislation.
  • Utilize coding systems effectively for patient records.
  • Manage chronic disease registries to ensure accurate tracking of patient health.
  • Establish and run well-woman and well-man health clinics.
  • Assist in minor surgical procedures as required.
  • Prioritize health concerns and intervene as appropriate.
  • Support the clinical team during emergencies.
  • Recognize and refer patients with mental health needs to appropriate services.
  • Implement vaccination programs for both adults and children.
  • Guide patients in the use of prescribed and over-the-counter medications within the scope of practice.
  • Collaborate with external agencies to ensure comprehensive support for vulnerable patients.
  • Delegate clinical tasks appropriately, ensuring safe practices.
  • Assist the clinical team with safeguarding matters in line with local and national policies.
  • Understand and apply local policies regarding substance abuse and addiction, referring patients as necessary.
  • Deliver opportunistic health promotion when suitable.

Additional Responsibilities

In addition to the primary duties, the Clinical Nurse Specialist may be asked to:

  • Lead audit initiatives, effectively utilizing the audit cycle.
  • Oversee the safe storage and disposal of medical supplies.
  • Mentor junior nursing staff, providing guidance and support.
  • Engage in local projects aimed at enhancing service delivery and patient care.
  • Participate in shared learning opportunities within the organization.
  • Continuously evaluate clinical practices in response to national policies and initiatives.
  • Contribute to the review of significant incidents, applying structured analysis methods.

Qualifications

Essential

  • Registered Nurse qualification.

Desirable

  • Postgraduate diploma or degree in Primary Care.

Experience

Essential

  • Experience in a primary care setting.
  • Experience in managing chronic diseases.

Desirable

  • Experience as a practice nurse or community nurse.

Clinical Knowledge

Essential

  • Wound care and suture/staple removal.
  • Electrocardiograms (ECGs).
  • Venepuncture skills.
  • Conducting new patient assessments.
  • Chaperoning procedures.
  • Requesting and processing pathology tests, advising patients accordingly.
  • Travel medicine expertise.
  • Knowledge of diabetes, hypertension, asthma, and coronary heart disease.
  • Immunization practices (routine, childhood, and travel).
  • Women's health services (cervical cytology, contraception, etc.).
  • Understanding of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to maintain accurate clinical records.
  • Ability to work within the scope of practice and recognize when to refer to GPs.
  • Knowledge of health promotion strategies.
  • Understanding of Patient Group Directions (PGDs) and related policies.

Desirable

  • Awareness of public health issues in the local community.
  • Understanding of broader health system challenges.
