Mental Health Community Nurse

We are seeking a highly skilled and motivated Registered Mental Health Nurse to join our North East Kent Home Treatment Team. As a key member of the multidisciplinary team, you will be responsible for assessing, planning, delivering, and evaluating care for patients with memory impairment and/or functional mental ill health.
Main Responsibilities- Manage a defined caseload of patients with functional and organic mental illness under supervision.
- Conduct comprehensive holistic assessments, taking into account relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors.
- Work collaboratively with other disciplines, patients, and their carers to agree a holistic person-centred care plan.
- Deliver and evaluate safe, competent, person-centred care in partnership with the service user and their carers to support recovery.
