Job Summary
We are seeking an experienced Practice Nurse to join our team at Two Rivers Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our practice.
Main Responsibilities
- Undertake health assessments, administer medication and treatments, and perform diagnostic tests.
- Provide and evaluate a range of health interventions to ensure that patients' health and social care needs are met.
- General health and well-being promotion, as well as creating individual, person-centred plans for patients with, or at risk of, a long-term condition.
- Manage and treat long-term conditions, in line with national and local policies and the practice needs.
- Review medication for therapeutic effectiveness, in accordance with evidence-based practice, as well as national and practice protocols, working with patients to support their adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately, to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
- Support and manage the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implement, and participate in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meet the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Run asthma, COPD, and diabetes clinics and reviews.
- New patient and health checks.

Person Specification
Qualifications
Essential
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- Competence to undertake asthma, COPD, and diabetic clinics and reviews, holding relevant certificates.
Desirable
- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, dressing and wound care, and recording spirometry.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives, and carers, as well as their colleagues at the practice.
- Knowledge, and experience of its application, in supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
