Healthcare Specialist

We are seeking a highly skilled and experienced paediatric nurse to join our Children's Continuing Care team as a Children's Continuing Care Assessor. This exciting opportunity will allow you to work collaboratively with healthcare providers, schools, and other stakeholders to assess the needs of children and young people with complex health needs.
The successful candidate will have a strong background in paediatric nursing, with experience in assessing and planning care for children with complex health needs. They will also have excellent communication and interpersonal skills, with the ability to work effectively with a range of stakeholders, including families, healthcare providers, and other professionals.
The role will involve undertaking individual Children's Continuing Care Assessments, working with providers of healthcare in the home and school settings, and informing the targeting of resources, monitoring, implementation, and evaluation of Packages of Care and Personal Health Budgets. The successful candidate will also be responsible for ensuring accurate and open communication and coordination with a range of organisations and individuals, researching and drafting correspondence and papers, and managing specific tasks, leading reporting and analysis across a range of specialties, functions, and projects.
About the Role- Undertake individual Children's Continuing Care Assessments in line with the National Framework
- Work with providers of healthcare in the home and school settings as required
- Inform the targeting of resources, monitoring, implementation, and evaluation of Packages of Care and Personal Health Budgets
- Ensure accurate and open communication and coordination with a range of organisations and individuals
- Support in the strategic development and oversight of local transition planning processes and planning meetings
- Current NMC Registration in Children's Nursing
- Further training or significant experience in project management, financial management, or supporting change management processes
- Experience and understanding of evaluating and measuring the performance of health services
- Comprehensive knowledge of project management and/or health information systems development
- Working knowledge of Microsoft Project ECDL
- A good understanding of the health and social care environment and roles and responsibilities within it
- Attention to detail combined with the ability to extract key messages from complex analysis
- Skills for managing projects ensuring they meet financial targets
- Clear communicator with excellent writing, report writing, and presentation skills
- Skills for nurturing key relationships and maintaining networks
- Evidence of success in efficient and effective project and programme management
