Key Responsibilities

  • Patient Engagement: Cultivate trusting relationships with patients by dedicating time to understand their priorities and facilitating their journey through the clinical pathway with personalized support.
  • Pathway Coordination: Oversee and monitor patients' journeys, collaborating closely with the multidisciplinary team to ensure their unique needs are addressed throughout.
  • Collaboration: Forge strong partnerships with community organizations and health and social care providers.
  • Outreach and Insights: Conduct direct outreach with patients, community groups, system partners, and healthcare professionals to provide comprehensive insights and enable targeted planning to improve health equity outcomes.
  • Caseload Management: Prioritize and manage a caseload based on the needs and support required by individuals.
  • Leadership and Improvement: Exemplify compassionate and inclusive leadership, fostering a culture of collective leadership within the organization.
  • Service Knowledge: Maintain a thorough understanding of the array of support services available within health and social care, guiding patients to the services that best meet their needs.
  • Referral Pathways: Identify new referral pathways and enhance existing ones, establishing strong connections with local voluntary and community sector organizations.
  • Support and Signposting: Assist patients in addressing issues affecting their health and well-being by directing them to appropriate support services.

Qualifications

Essential:

  • Prior experience in a health coaching role or a similar public health position, community development, or social prescribing at a degree level or equivalent.

Desirable:

  • Degree or equivalent in health, social care, or community development.
  • Accredited health coaching qualification or training in motivational interviewing.
  • Relevant awards in supporting behavior change in health and well-being.

Other Requirements

Essential:

  • Ability to meet travel requirements for the role, which may involve visiting various sites and community locations.
  • Willingness to work across different organizational premises as necessary for the role.
