Job Overview

Key Responsibilities

The clinical pharmacist will primarily manage the prescription request process, conduct comprehensive face-to-face medication evaluations, particularly for patients with multiple medications, and engage in quality initiatives such as sharing best practices and conducting audits across the network.

The Clinical Pharmacy team will spearhead the development and management of medication optimization services within the network's practices. Responsibilities include leading quality improvement efforts, overseeing clinical audits, and managing medication safety and antibiotic stewardship initiatives. Additionally, support will be provided for patients with chronic conditions.

Job Duties

Management of Long-Term Conditions and Elderly Care

  • Oversee chronic disease management, including hypertension, Type 2 Diabetes, Asthma, and COPD.
  • Conduct medication reviews for older adults with frailty.
  • Perform clinical medication assessments for patients with multiple health issues and implement prescribing changes as an independent prescriber.

Care Home Medication Management

  • Execute clinical medication reviews for care home residents and provide recommendations to senior pharmacists and healthcare professionals.
  • Collaborate with care home staff to enhance the safety of medication ordering and administration.

Risk Management and Quality Improvement

  • Conduct audits and medication adjustments in line with practice and ICB priorities.
  • Support prescribing safety, clinical audits, and local enhanced services.
  • Focus on improving disease registers and quality outcomes.

Support for Hospital Discharges and Unplanned Admissions

  • Carry out medication reviews based on requests from hospital specialists.
  • Manage acute medication requests and ensure proper medication management upon discharge.

Patient Support and Clinical Reviews

  • Provide advice on minor ailments as appropriate.
  • Handle daily prescription requests, making clinical decisions and prescribing as necessary.
  • Offer clinical guidance on medications to both patients and practice staff.
  • Maintain a telephone support line for patient inquiries regarding their medications.

Service Development and Quality Improvement

  • Share the workload related to medications across the practice to support other healthcare professionals.
  • Enhance the safety and quality of prescribed medications.
  • Provide comprehensive support on medication-related issues to the entire practice team.
  • Lead medication reconciliation during transitions between care providers.
  • Implement changes in medication management as per MHRA alerts and local guidelines.
  • Promote evidence-based practices to prevent infections and reduce inappropriate antibiotic use.

Communication and Collaboration

  • Recognize the roles of colleagues in patient care and utilize effective communication to foster cooperation.
  • Work collaboratively within the team and with external partners to enhance patient care.
  • Develop and maintain strong relationships with local healthcare services.

Quality Service Delivery

  • Organize and manage personal workload, including audits and training for practice team members.
  • Understand national priorities and their implications for service delivery.
  • Stay updated with relevant standards of practice and maintain professional registration.
  • Engage in continuous professional development and participate in relevant meetings.
  • Comply with all organizational and statutory requirements related to health and safety, confidentiality, and clinical governance.

Education and Training

Provide training to practice teams on therapeutics and medication optimization.
