Job Overview

Regular interaction with patients is expected, ensuring that the workload is efficiently managed across the practice team to meet patient needs effectively.

Key Responsibilities

The clinical pharmacists will primarily focus on overseeing the prescription request process, conducting comprehensive face-to-face medication assessments, particularly for patients dealing with multiple medications, and engaging in deprescribing practices. They will also participate in network quality initiatives, sharing best practices and conducting cross-network audits.

The Clinical Pharmacy team will be responsible for developing and managing medication optimization services within the network practices. Leadership in quality enhancement and clinical audits will be essential, alongside managing aspects of medication safety and antibiotic stewardship. Support for patients with chronic conditions will also be a key component of this role.

Responsibilities Include:

  • Managing chronic diseases such as hypertension, Type 2 Diabetes, Asthma, and COPD, with a focus on polypharmacy reviews.
  • Conducting medication assessments for elderly patients with frailty.
  • Performing clinical medication evaluations for patients with multiple health issues and implementing necessary prescribing adjustments as an independent prescriber.
  • Collaborating with care home staff to enhance the safety of medication ordering and administration.
  • Conducting audits and medication switches in alignment with practice and Integrated Care Board (ICB) priorities.
  • Providing medication reviews in response to requests from hospital specialists.
  • Managing medication requests for acute items and overseeing medication management at hospital discharge.
  • Offering clinical advice and support regarding medications to practice staff and patients.
  • Leading initiatives to improve the quality and safety of prescribed medications.
  • Ensuring effective communication and collaboration with various healthcare professionals to enhance patient care.

Professional Development

The role includes providing education and training to practice teams on therapeutics and medication optimization, ensuring continuous professional growth and adherence to relevant standards of practice.

Qualifications and Experience

Essential:

  • A minimum of 2 years of post-qualification experience as a pharmacist.
  • Completion of an undergraduate degree in pharmacy.
  • Progressing towards an independent prescribing qualification.

Desirable:

  • Holding an independent prescribing qualification.
  • Clinical Diploma.
