Job Overview

We are seeking a highly skilled and experienced District Nurse Team Leader to join our innovative and dedicated team at Oxford Health NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering outstanding care to the communities of Oxfordshire.

Main Responsibilities
  • Lead a team of community nurses in providing high-quality, evidence-based care to patients in their own homes.
  • Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals.
  • Collaborate with other teams and services to ensure seamless care transitions and optimal patient outcomes.
  • Contribute to the development and implementation of community nursing strategies, policies, and procedures.
  • Provide clinical supervision and mentorship to junior staff members.
Requirements
  • Registered General Nurse (RGN) with current NMC registration.
  • Level 6 study or higher.
  • Formal management and/or leadership experience within healthcare or extensive clinical experience as a Registered Nurse within a District Nursing team.
  • Community nurse experience and knowledge of community nursing practices.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work autonomously and as part of a team.
