Position Overview
Leicestershire Partnership NHS Trust is collaborating with Adult Social Care to enhance our Home First service alongside essential Community Therapy.
This is an exciting opportunity to join a service that integrates health and social care, delivered by community-based nursing, therapy, and adult social care teams. In this role, you will have the chance to:
- Assist individuals in maintaining their independence at home during health or social care emergencies, avoiding unnecessary hospital or care home admissions.
- Facilitate swift hospital discharges and provide rehabilitation and reablement support to restore patients' well-being and autonomy.
- Deliver high-quality therapy services focused on reablement, rehabilitation, and recovery within patients' homes.

Our community therapy team offers regular in-service training and opportunities for skill development through collaboration with experienced Clinical Specialists and Operational Leads, as well as access to relevant courses.
We also promote flexible and blended working arrangements.
We have multiple positions available for Clinical Rehabilitation Physiotherapists across the region, both full-time and part-time.
Main Responsibilities
We are seeking Clinical Rehabilitation Physiotherapists to deliver an effective and efficient therapy service. This role requires a motivated, resilient, and proactive individual who can prioritize and manage their own caseload, demonstrate initiative, and work both independently and collaboratively within a team. Strong communication and organizational skills are essential, along with the ability to adapt to dynamic and high-pressure environments.
You will collaborate with Occupational Therapists and Technical Instructors to conduct comprehensive functional assessments and interventions aimed at helping patients achieve maximum independence. Our therapy teams operate in an integrated manner, encouraging therapists to work synergistically to achieve optimal patient outcomes.
We are looking for determined and self-motivated professionals who can enhance patient care and advocate for therapy services within the Trust.
It is essential to possess a full UK driving license and have access to a vehicle for work-related purposes.
