About the Role

We are seeking an experienced and organized individual to join our Proactive Care Team as an Administrative Support Officer. The successful candidate will provide administrative support to the team, ensuring the smooth delivery of services to vulnerable patients and those with complex physical or mental health needs.

Main Responsibilities

  • Manage and triage incoming referrals into the Proactive Care Team
  • Book and plan clinical appointments for home visits
  • Update clinical notes and prepare documentation for Nurse Appointments
  • Complete onward community referrals as required
  • Provide advice and signposting to service users, carers, and professionals
  • Coordinate clinical attendance at the Practice White Board Meetings
  • Support the clinical team with administrative tasks and meeting minutes
  • Complete audits as required

Person Specification

We are looking for an individual with experience in a primary care or healthcare setting, who is able to work independently and as part of a team. The successful candidate will have excellent communication and organizational skills, with the ability to maintain confidentiality and work in a fast-paced environment.
