GMMH is seeking an experienced Perinatal Outreach Practitioner to join our Specialist Perinatal Outreach Team. The successful candidate will provide outreach services across Greater Manchester, supporting individuals in the perinatal period.

Main Responsibilities
  • Assess and formulate care plans for patients, including discharge planning within agreed protocols.
  • Provide support to complex cases, ensuring the best possible care for mothers and babies.
  • Work closely with the ward and community multidisciplinary team to identify patients who would benefit from outreach services.
  • Manage and triage referrals into the ward and outreach team.
  • Act as bed manager for Andersen Ward Mother and Baby Unit.
