About the Role

This is an exciting opportunity for an experienced Nurse to join our Community Treatment Team as a Band 5 Community Practitioner. The successful candidate will have the opportunity to develop their skills and knowledge in a specialist field, working closely with a multidisciplinary team to deliver high-quality care and treatment to individuals with moderate to severe mental health problems.

Main Responsibilities

  • Assess and manage the care needs of service users with complex mental health conditions
  • Develop and implement individualized care plans in collaboration with service users and their families
  • Work closely with other agencies and services to provide effective care and treatment
  • Contribute to the development of the team and the service, identifying areas for improvement and implementing changes as needed
  • Participate in supervision and continuing professional development to maintain and enhance skills and knowledge

Requirements

  • First Level Registration RMN/RNMH/RNLD Qualification
  • 12 months post-registration experience and completed preceptorship
  • Experience of working with service users and carers in the delivery of treatment plans
  • Ability to use IT systems and comprehensive continuous assessment of skills
  • Adaptability, flexibility, good communication skills, and a commitment to multi-agency team work
