About the Role

CJP Outreach Services is seeking a skilled and compassionate Community Care Specialist to join our dynamic team. As a key member of our outreach services, you will play a vital role in supporting individuals with complex needs in the Brighouse, Halifax, and Bradford areas.

Key Responsibilities
