Job Title: Social Worker - 16-25 Disabilities Team

Location: South Gloucestershire
Job Description:
We are seeking a qualified Social Worker to join our 16-25 Disabilities Team in South Gloucestershire. As a Social Worker in this team, you will work under adult legislation to consider eligibility under The Care Act and provide service provision as appropriate.
Key Responsibilities:
- Assess and plan for the needs of young people transitioning from Children's Services or who have recently done so.
- Carry out assessments under The Care Act, seeking funding for care packages and reviewing services with a multi-agency approach as appropriate.
- Help families understand that their young people are now adults and are able to make their own choices for greater independence.
- Navigate the transition process into adulthood with different professionals and legislation involved.
Requirements:
- Qualified Social Worker with relevant Social Work England registration to practice.
- Good understanding of asset and strengths-based approaches to social work.
- Specialist up-to-date knowledge of the service area of Adult care and disability.
- Ability to demonstrate knowledge and experience of using relevant legislation, regulations, and codes of practice.
- Understanding of the 3 Conversations way of working.
- Familiarity with Mosaic.
