Job Description

Role Overview

Randstad Care is seeking a qualified Social Worker to join our team in South Gloucestershire Council's 16-25 CWD team. As a Social Worker in this team, you will work under adult legislation to consider eligibility under The Care Act and provide service provision as appropriate.

Key Responsibilities

  • Manage a caseload of young people transitioning from Children's Services or who have recently done so, planning for their next stage in life, including changes around education, provision, budget, and independence.
  • Carry out Assessments under The Care Act, seeking funding for care packages and reviewing services with a multi-agency approach as necessary.
  • Help families understand that their young people are now adults and can make their own choices for greater independence, navigating the transition process into adulthood with different professionals and legislation involved.

Requirements

  • Qualified Social Worker with relevant Social Work England registration to practice.
  • Good understanding of asset and strengths-based approaches to social work.
  • Specialist knowledge of adult care and disability, including The Care Act, Mental Capacity Act, EHCP Code of Practice, Safeguarding Adults Policies and Procedures, and Deprivation of Liberty responsibilities.
  • Familiarity with Mosaic and the 3 Conversations way of working.
  • Ability to assess, plan, provide, and review for the most vulnerable young adults in the community, including risk assessment and management.
  • Understanding of possible funding changes from the 18th birthday, including CHC.

Qualifications and Experience

  • Social Work England Registered (SWE).
  • Enhanced DBS.
  • Social Work Degree or equivalent.
