About the Role

We are seeking an experienced Occupational Therapist/Advanced to join our team in the South Gloucestershire Reablement Service. As a key member of our team, you will play a vital role in enabling individuals to achieve independence and maintain their quality of life.

Key Responsibilities
  • Assess and develop individualised plans to support people in achieving their goals and maintaining independence.
  • Work closely with assessing practitioners and reablement providers to ensure that individuals receive the best possible support.
  • Provide expert advice and guidance on equipment, assistive technologies, and housing adaptations to support individuals in maintaining their independence.
  • Hold a caseload and work closely with colleagues in the locality teams to support reablement referrals.
  • Advise on strategies and resources to enhance reablement outcomes, including training and signposting.
  • Work closely with Disabled People and their Carers to provide information on services available, including those under the Care Act.
  • Be responsible for making recommendations on budget spend to ensure maximum value is delivered.
Requirements
  • Professional Occupational Therapy Qualification (Dip COT, BSc Honours OT, or equivalent) and current State Registration with the Health Professionals Council.
  • Experience of working in a hospital or community setting, or in a voluntary or agency providing Occupational Therapy Services.
  • Ability to demonstrate theoretical and applied knowledge relating to Disabled People and their needs.
  • Up-to-date knowledge of related legislation relevant to Social Services Occupational Therapy and legislation supporting this.
  • Experience of managing complex cases and applying excellent assessment skills in practice.
  • Excellent time management skills to work efficiently and meet service requirements.
