About the Role

We are seeking an experienced and skilled Advanced Practitioner to join our team at Barnsley Primary Care Network. As an autonomous practitioner, you will work independently and in conjunction with other healthcare professionals to assess, diagnose, and treat patients attending the i-Heart service.

Key Responsibilities:

  • Assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions.
  • Initiate direct referrals as appropriate and work with other healthcare professionals to provide comprehensive care.
  • Develop and manage highly specialized programmes of care and care packages.
  • Contribute to the strategic development of i-Heart and additional public health services to meet patient and population needs.
  • Work with the extended primary healthcare team to provide education and training to other staff and students.

Requirements:

  • Masters level (or working towards) in Advanced Practice Studies, plus supplementary postgraduate level courses.
  • Counselling skills and experience of interagency working and adult/child protection.
  • Minimum of 3 years post-Advanced Practice experience within the clinical field.
  • Able to demonstrate a depth and breadth of current issues and policies within the NHS and have the ability to translate into practice.
