As an autonomous practitioner, you will work independently and in conjunction with other healthcare professionals to assess, diagnose, and treat patients attending the i-Heart service. Your duties will include visit triage, home visits to housebound patients, and seeing on-the-day patients for acute and minor illnesses in a clinical setting. You will work as part of a multi-skilled, professional team alongside GPs, Physician Associates, and other ACPs.

Responsibilities", "ul": ["Assesses, diagnoses, and treats patients attending the i-Heart service with a range of acute, non-acute, and chronic medical conditions.", "Assesses, diagnoses, and treats patients in surgery, hubs, or their own home who require acute medical attention and refer to appropriate agency.", "Directly admits patients to secondary care hospitals in acute medical need.", "Refers patients to other medical specialties for assessment if deemed appropriate.", "Has undertaken x-ray training as required by hospitals trust.", "Can interpret a range of diagnostic tests and perform routine clinical procedures e.g. cytology, venepuncture, blood pressure monitoring, etc.", "Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.", "Masters level (or working towards) in Advanced Practice Studies, plus supplementary post-graduate level courses: ", "Counselling skills.", "Minimum of 3 years post Advanced Practice experience within the clinical field.", "Able to demonstrate a depth and breadth of current issues and policies within the NHS and have the ability to translate into practice.", "Autonomous practitioner working independently and in conjunction with other health care professionals.", "Understanding and experience of interagency working and adult/child protection.", "Evidence of continued professional development.", "Experience of Clinical Governance and risk assessments."]}
