About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at Modality Partnership. As a Clinical Pharmacist, you will play a key role in providing clinical pharmacy services to patients in a primary care setting.

Key Responsibilities
  • Provide clinical pharmacy services to patients, including medication reviews, disease state management, and patient education.

Requirements
  • Qualified pharmacist with a PgCert.

  • Effective verbal and written communication skills.
  • Effective time management skills to balance work and education requirements.
