As a key member of our team, you will be responsible for managing the care of patients with complex needs, including those with dementia, delirium, mental health illnesses, or a learning disability.
Main Responsibilities- Provide leadership and clinical guidance to the Enhanced Care Team
- Develop and implement care plans that meet the individual needs of patients
- Collaborate with medical staff to ensure seamless care transitions
- Conduct regular audits to ensure high standards of care and patient safety
- Support the development of team members through coaching and mentoring
