We are seeking a highly skilled and dedicated Lead Practitioner to join our team. As a key member of our care team, you will be responsible for delivering high-quality support to our service users, meeting their individual needs and exceeding their expectations.
Key Responsibilities:
- Conduct initial assessments and follow-up reviews of service users, creating and implementing personalized support plans.
- Manage and maintain accurate records, adhering to data protection regulations.
- Provide leadership and on-the-job training to staff, promoting a culture of excellence and continuous improvement.
- Take charge of shift duties, ensuring seamless service delivery and staff supervision.
- Maintain confidentiality and professionalism at all times.
- Develop and implement risk management plans to ensure a safe and supportive environment.
Requirements:
- Hold a minimum SVQ3 qualification.
- Proven experience working with individuals with learning disabilities and/or forensic histories.
- Knowledge of SSSC standards and experience in organizational planning and record-keeping.
- Understanding of social exclusion and its causes and effects.
- Ability to work independently and as part of a team.
