Job Summary

We are seeking a skilled and compassionate Care Home Nurse Specialist to join our team at Black Country Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with mental health needs in care home settings.

Key Responsibilities
  • Provide comprehensive assessments and triage of mental health needs, using evidence-based outcome measures and sound knowledge base.
  • Work collaboratively with service users, carers, and colleagues to develop effective strategies for promoting understanding and management of mental health problems.
  • Offer information, advice, education, and health promotion to service users, carers, and the wider community about mental health and available options.
  • Critically analyze service user information and formulate individual care plans using evidence-based brief interventions.
  • Participate in the development and delivery of therapeutic and support groups in response to local need.
  • Attend and actively contribute to multidisciplinary team meetings and caseload supervision to ensure high-quality clinical work.
  • Facilitate the stepped care model and clinical pathways for individuals experiencing enduring mental health problems.
Requirements

To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as a sound knowledge base in mental health and care home settings. You will also need to be able to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care.
