As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality, person-centred care to adults with learning disabilities, autism, and complex behavioural needs.

Key Responsibilities:

  • Maintain a high-quality service by working in partnership with colleagues and stakeholders to manage and support all services.
  • Develop and maintain positive relationships with families and other professionals to ensure a collaborative approach to care.
  • Assist the Registered Manager with reviewing budgets, staff rotas, complaints, and relationship building with external regulatory bodies.
  • Provide support, supervision, development, and mentoring to all staff, including appraisal and supervision.
  • Contribute to the development of new policies and practices within the Home and maintain systems work within and adhere to the CQC standards and regulations.
  • Cover the manager during absences.

Requirements:

  • Willingness to undertake the Skills for Care Induction.
  • Qualified QCF Level 3 or NVQ Level 3/4.
  • Previous experience working in a residential/supported living setting, including supervisory responsibilities, supporting Adults with Learning Disabilities.
  • Person-centred planning, care, and support planning, and risk assessment.
