Rate: Up to £31 per hour

We are seeking a skilled Physiotherapist to join our Multi-Disciplinary Rapid Response Therapy team in Stockport. As a Rapid Response Physiotherapist, you will work in a dynamic team that prevents unnecessary hospital admissions by providing care to patients in their homes at short notice.

Key Responsibilities:

  • Provide physiotherapy services to patients in their homes
  • Work collaboratively with the multi-disciplinary team to develop and implement patient care plans
  • Conduct assessments and evaluations to identify patient needs and develop effective treatment plans
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

Requirements:

  • Experience working in a rapid response team or acute and community settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work varied shifts, including evenings and weekends
