Job Description:
Job OverviewProcare Health Ltd is seeking a highly motivated Registered Nurse to join our team of healthcare professionals in the community. As a Nursing Home Nurse, you will be responsible for delivering high-quality nursing care to patients in their own homes, working autonomously as delegated by and under the direction of the District Nurse.
Main Responsibilities- Assess and implement person-centered care for a caseload of patients, including those with long-term conditions, complex care needs, and end-of-life care.
- Use electronic patient-held record systems, such as EMIS, to record care and treatment.
- Develop and maintain relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
- Work collaboratively with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses to provide comprehensive care.
- Participate in training and development opportunities to enhance skills and knowledge in community nursing.
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
- Educated to diploma or level 6/degree level in a relevant area.
- Supervisor/Mentorship qualification within the first 2 years of post.
- Evidence of continuing professional development.
- Ability to work across a seven-day week between 8 am and 8 pm as per service requirements.
