The post holder will work as part of a team to deliver nursing care and treatment autonomously, as delegated by and under the direction of the District Nurse.
The post holder will visit adult patients in their own homes who have identified nursing needs, including those with long-term conditions, complex care requirements, and end-of-life care.
The post holder will use an electronic patient-held record system, EMIS, and will be provided with a work iPhone and iPad for mobile working.
The community nurse will utilize a range of nursing skills, including venepuncture, wound care, catheterization, syringe drivers, end-of-life care, and symptom control.
On successfully attaining all required competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.

We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and healthcare assistants, assessing, implementing, and evaluating person-centered care for a caseload of patients.
The post holder will work under the direction of a District Nurse, undertaking venepuncture, wound care, catheterization, syringe drivers, end-of-life care, and symptom control.
The post holder will be available to work Monday to Sunday between 8 am and 8 pm and will be required to travel to patients' homes frequently throughout the day across the locality and across Guildford and Waverley.
