Job Summary

We are seeking a skilled and compassionate District Nurse to join our Community Team on a 9-month fixed-term contract. As a key member of our team, you will work in partnership with individuals and their carers to develop personalized plans of care, proactively monitoring and anticipating changing needs, and coordinating care across the health and social care system.

About the Role

The successful candidate will hold overall responsibility for a designated caseload, managing and delegating the delivery of care by the integrated team. You will be expected to apply advanced assessment skills to provide clinical care and health-promoting interventions, helping to avoid unnecessary hospital admissions.

Key Responsibilities
  • Develop and implement personalized care plans for individuals and their carers
  • Proactively monitor and anticipate changing needs, coordinating care across the health and social care system
  • Manage and delegate the delivery of care by the integrated team
  • Apply advanced assessment skills to provide clinical care and health-promoting interventions
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Requirements
  • NMC Registered Nurse
  • Post-registration study or qualification
  • Mentorship or equivalent/practice assessor
  • Leadership/management course or willingness to work towards
  • District Nurse qualification
