Position Overview

Join Derbyshire Healthcare NHS Foundation Trust as a Senior Practitioner focused on Older Adults

The Erewash Older Adult Community Mental Health Team (CMHT) and Living Well Team are seeking a dedicated Band 6 Specialist to become part of their collaborative and nurturing teams.

The Erewash Older Adult team is known for its supportive environment, ensuring that all team members feel appreciated, encouraged to explore professional curiosity, and invited to share their insights and ideas.

The role of the Senior Practitioner will bridge both the Older Adult and Living Well Teams, facilitating a partial integration to guarantee that older individuals in the community can access the Living Well ageless services.

Key Responsibilities

The ideal candidate will possess experience in working with older adults facing functional mental health challenges within a community context.

This newly established position, hosted by the Older Adults CMHT, will involve aiding transitions into the Living Well Team, assessing the needs of older adults with mental health concerns residing in the local area, and crafting personalized support plans to empower individuals to lead fulfilling lives.

This role will require collaboration with community colleagues to ensure timely and appropriate service delivery by the right professionals.

Supported by the Older Adult CMHT, the role will also engage with the Living Well Team to fulfill the ageless component of the Community Mental Health Framework.

The Living Well initiative is a locality-based, multi-agency team that provides integrated support by centering the individual’s needs. A fundamental aspect of this framework is to assist those who may not have previously qualified for secondary mental health services.

Travel Requirements

The successful candidate must be able to travel across the designated geographical area.

Job Responsibilities

The post holder will be clinically accountable for delivering a flexible and responsive local service, advancing an Integrated Wellbeing Pathway in the assigned locality, which effectively supports the implementation of the Community Mental Health Framework. The post holder will collaborate with colleagues, key stakeholders, and local community members to develop services that address the mental health needs of the population.

The role will require the application of a diverse skill set, building upon and enhancing expertise while actively seeking guidance and collaboration on issues that may extend beyond their immediate practice area. Adherence to professional practice guidelines, including HCPC/NMC standards or equivalent professional bodies, is essential.

The responsibilities will include conducting comprehensive person-centered psychosocial assessments, risk evaluations, and formulating responses to presenting mental health needs. The practitioner will establish therapeutic relationships, co-create care plans with individuals, and support them through challenging times by empowering them to take charge of their health.

This role also involves motivating individuals to enhance their mental health by fostering self-care skills, resilience, and effective use of community resources. A crucial aspect of this evolving role will be to cultivate and maintain relationships with community partners, including statutory organizations, Voluntary, Community, and Social Enterprise (VCSE) entities, and the local communities served. Listening to and addressing the perspectives of lived experience is vital for the development of this service.

Qualifications

  • Relevant qualifications for the position (Diploma in Nursing/Degree in Mental Health Nursing or Degree in Occupational Therapy)

  • Post-qualification experience in the relevant field
  • Experience working with older adults with functional mental health diagnoses
  • Some experience in community engagement
  • Ability to meet travel requirements of the position
  • Strong communication skills
  • In-depth understanding of mental health disorders
  • Knowledge and application of theoretical models of practice and core professional assessments relevant to their background
