Social Care Specialist

**Job Summary**
Derbyshire County Council is seeking a skilled Social Care Practitioner to join our team. As a Social Care Practitioner, you will work with adults in the community and residential settings, providing support and advice to clients and carers.
**Key Responsibilities**
- Conduct assessments and create person-centred support plans to enable individuals to meet their outcomes.
- Work in a team setting, contributing to safeguarding investigations as needed.
- Collaborate with other professionals, service users, and their families and carers in a multi-disciplinary partnership.
- Travel to various locations, ideally with access to your own transport.
**Requirements**
- Experience in adult social care, with a focus on community and residential settings.
- Ability to work within budgets and knowledge of outcome-focused assessments and reviews.
- Strong communication and interpersonal skills.
