About the Role

We are seeking a motivated and enthusiastic Assistant Practitioner to join our Community Nursing team in Bedfordshire. As a key member of our team, you will be responsible for providing high-quality care to patients in their own homes.

Key Responsibilities

  • Work corporately within our Primary Care Team to provide high-quality care to housebound residents.
  • Support the Community Nurses and the wider MDT to deliver responsive and high-quality care.
  • Manage a caseload of patients and prioritize work to meet their needs.
  • Develop and maintain effective relationships with patients, carers, and family members.

Requirements

  • Essential qualifications: ONVQ III, GCSE Maths or equivalent, Diploma level relevant qualification or equivalent experience.
  • Desirable experience: working with community health care teams, experience of working with people on a curative and or palliative pathways, experience of health coaching and promoting independent living.
