Job Summary

Band 4 Assistant Practitioner Nursing

We are seeking a motivated and enthusiastic Band 4 Assistant Practitioner to join our Community Nursing team in Bedfordshire. This substantive post is for the Mid Beds Community Nursing Team.

This highly respected team delivers responsive high-quality care to patients in their own homes.

We are passionate about patient-centered care and support our team to lead and deliver excellent and compassionate care.

Main Duties of the Job

You will need to be a motivated and enthusiastic person, who has excellent communication, IT literate, and problem-solving skills, alongside a passion for patient-centered care. You will work corporately within our Primary Care Team and will be supporting the Community Nurses and the wider MDT, to provide high-quality care to housebound residents who live with a wide range of complex health conditions.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important.

Job Responsibilities

The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

A full induction program will be provided.

Education/Qualification/Training

Essential

  • oNVQ III
  • oGCSE Maths or equivalent
  • oDiploma level relevant qualification or equivalent experience

Experience

Essential

  • oDemonstrates appropriate emotional self-regulation and maintains professional boundaries at all times
  • oGood experience of working with community healthcare teams
  • oExperience of working with people on a curative and or palliative pathways
  • oExperience of working with the public demonstrating good customer care skills
  • oExperience of health coaching and promoting independent living
  • oExperience of working with people with physical health requirements
  • oExperience of managing caseload of patients
  • oExperience of working with people with learning difficulties and or mental health issues
  • oExperience of working without direct supervision

Knowledge and Skills

Essential

  • oDemonstrates good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
  • oDemonstrates good understanding of the Mental Capacity Act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance as required
  • oAbility to prioritise work and work in a time-managed and effective way to meet patients' appointments and demands of the role
  • oComprehensive clinical skills (, venepuncture, Doppler, wound care, continence care) etc
  • oAbility to manage a caseload of patients
  • oBasic ability to use Microsoft packages - Word, Excel - and databases.
  • oAble to support clients, carers, and family during times of emotional distress
  • oBe able to undertake frequent standing, kneeling, crouching, hoisting, and other moving and handling procedures
  • oRegularly move and handle clients that may require moderate physical effort, in confined spaces and often unsuitable environments, using relevant practical experience, equipment, and skills and enable optimum safety and best practice.
