About the Role

We are seeking an experienced Occupational Therapist to join our Acute Frailty Service as a Specialist Frailty Occupational Therapist. This is an exciting opportunity to work in a rapidly developing service that provides comprehensive geriatric assessments to frail patients presenting to the acute areas of Calderdale and Huddersfield NHS Foundation Trust.

Key Responsibilities

  • Assess frail patients across CHFT, including the new emergency department, medical assessment unit, and Frailty Same Day Emergency Care unit (FSDEC).
  • Utilize core occupational therapy skills to support the complex needs of frail patients.
  • Work within a multi-disciplinary team to provide holistic assessments, meeting the needs of patients with a strong focus on the home first approach.
  • Develop individual treatment plans, expanding knowledge with skill sharing competencies.
  • Contribute to service development, working collaboratively with health and social care colleagues.

Person Specification

  • Registered with HCPC.
  • Relevant Degree/Diploma.
  • Recent and extensive clinical experience working with frail patients.
  • Awareness of relevant Legislation, NSF'S/Local and National initiatives.
  • Excellent Clinical Reasoning skills in situations of complex health and social needs.
