Job Title: Specialist Occupational Therapist or Physiotherapist

We are seeking a highly skilled and experienced Specialist Occupational Therapist or Physiotherapist to join our Acute Frailty Service at Calderdale and Huddersfield NHS Foundation Trust.

About the Role

This is a challenging and exciting opportunity to work within an evolving integrated multidisciplinary team, providing a comprehensive geriatric assessment to frail patients presenting to the acute areas of CHFT.

As a Specialist Occupational Therapist or Physiotherapist, you will be required to assess frail patients across CHFT, including the new emergency department, medical assessment unit, and Frailty Same Day Emergency Care unit (FSDEC).

You will utilise your core occupational therapy skills to support the complex needs of frail patients, working within a multi-disciplinary team to provide holistic assessment meeting the needs of our patients with a strong focus on the home first approach.

Key Responsibilities
  1. Act as a specialist clinician for the Acute Frailty Service within the Acute Medicine directorate.
  2. Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up-to-date records.
  3. Manage your own specialist caseload, demonstrating excellent skills in effective prioritisation in a fast-paced environment.
  4. Provide and develop the core components of the specialist therapy role including clinical practice, management, and education.
  5. Act as a clinical role model ensuring best practice in all assessments.
  6. Support the Team Leader in the day-to-day running of the Acute Frailty service.
  7. Advise in the wider MDT on the delivery of excellent care for the frail patient.
  8. Participate in the assessment, planning, and implementation of individualised treatment plans for frail older adults.
  9. Recognise changes in patients' conditions which require intervention.
  10. Advocate for patients following holistic assessment.
Clinical Duties
  1. Effectively identify patients living with frailty.
  2. Provide specialist therapy input to patients living with frailty.
  3. Complete Comprehensive Geriatric Assessments on patients presenting to CHFT.
  4. Assess, plan, and implement specialist therapy interventions.
  5. Maintain accurate, appropriate, and concise patient records.
  6. Provide relevant and clear information, education, and advice for patients, carers, and families.
  7. Act as the patient's advocate, supporting their involvement in decision-making.
  8. Provide specialist advice and information to professional colleagues, patients, and carers.
  9. Participate in risk assessment and supporting implementation of strategies to minimise risk.
  10. Establish and maintain effective communication.
  11. Delegate tasks and skill share appropriately.
  12. Provide leadership, guidance, and advice to staff.
  13. Participate in clinical meetings as required.
  14. Effectively manage a rapidly changing and complex caseload.
  15. Work collaboratively with health and social care community and acute colleagues.
  16. Ensure effective care pathways and transfers of care.
  17. Implement and review clinical standards relevant to the work undertaken.
  18. Work across a 7-day service with a variable shift pattern.
