We are seeking an experienced Mental Health Practitioner to join our Intensive Support Service team. As a key member of the multidisciplinary team, you will work closely with patients, families, and carers to provide specialist assessment, treatment, and support.

Key Responsibilities:
  • Provide high-quality, recovery-focused care to individuals with complex mental health needs
  • Work collaboratively with the multidisciplinary team to develop and implement care plans
  • Conduct assessments, risk assessments, and reviews to ensure effective patient care
  • Develop and maintain relationships with patients, families, and carers to promote independence and well-being
  • Participate in training and professional development to maintain expertise and stay up-to-date with best practices
