The East Lynne Medical Centre is seeking a highly skilled and dedicated Cancer Care Specialist to join our team. As a key member of our Primary Care Network, you will play a vital role in improving the health and wellbeing of patients within our network through improved screening, holistic assessment, and proactive management of cancer conditions.
Key Responsibilities- Identify and coordinate the implementation and evaluation of treatment/interventions and care for patients with complex needs.
- Assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Liaise with wider MDT and support the management of both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support the completion of Advanced care planning where appropriate including My care choice register, RESPECT forms and PPC documentation.
- Support practices with cancer patients that may rapidly decline and facilitate the right services to meet the patients needs.
- Work with other MDT to provide a holistic approach to the management of individuals needs for example referral to social prescribers or occupational therapists.
- Ensure accurate and legible notes of all consultations and treatments are recorded promptly in the patients notes on SystmOne/EMIS.
- Maintain infection control standards in varied and challenging environments in line with the organisations infection control policy and procedural guidance.
- Ensure the safety of patients, relatives, and colleagues and to report all incidents and near misses in line with the risk management policy.
- Maintain up-to-date knowledge of safeguarding adult and children guidance to ensure that those at risks of abuse/suffering abuse are identified and the necessary process to protect them is followed.
- Meet responsibilities as an employee as set out in the organisations corporate and clinical policies.
- Registration with the Nursing & Midwifery council
- Be able to carry out the duties of this post
- Minimum 3 years post registration experience
- Evidence advanced clinical skills and knowledge
- Demonstrate understanding of personalised care approach
- Demonstrate management of patients with complex needs
- Knowledge of public health issues relating to the local population
- Knowledge/understanding of the Primary Care Network Direct Enhanced Service, local ICB vision/values and how primary care fits in with the NHS Long Term Plan
- Good understanding of current issues within in the wider NHS
- Demonstrates professional pride.
- Able to manage own time and prioritise work effectively.
- Able to work well with others and deal with the challenges of ever changing and demanding situations.
- Excellent communication skills
- Ability to manage high pressure situations clinical crisis
- Willingness to work flexibly in order to deliver clinically effective and cost-effective healthcare.
- Evidence/demonstration of the sound clinical knowledge of a wide range of long-term/chronic conditions
- Understanding of the Quality Outcome & Investment and Impact Framework (QoF & IIF)
- Willingness to learn and develop.
- Act as an autonomous practitioner whilst working within their scope of practice.
