Salary: £37,038 - £44,814 per annum

Hours: Full-time, 36 hours per week

Benefits: Relocation allowance of up to £10,000, Retention Bonus of £1,000 for every completed year in service

Job Overview:

Supply Care Solutions is seeking a skilled and confident Social Worker to join our Adult Services team. As a Social Worker, you will play a vital role in delivering high-quality care and support to vulnerable adults in our community.

You will work collaboratively with individuals, families, and other professionals to assess needs, develop and implement care plans, and provide ongoing support to ensure the best possible outcomes.

We are looking for a Social Worker who is passionate about making a difference in the lives of others, has excellent communication and interpersonal skills, and is able to work effectively in a fast-paced environment.

Key Responsibilities:
  1. Assess the needs of vulnerable adults and develop and implement care plans to ensure their safety and well-being.
  2. Work collaboratively with individuals, families, and other professionals to provide ongoing support and guidance.
  3. Conduct regular reviews and assessments to ensure that care plans are effective and meet the changing needs of service users.
  4. Provide emotional support and guidance to service users and their families, and signpost them to other services and resources as needed.
  5. Work closely with other teams and agencies to ensure that service users receive a seamless and coordinated service.
  6. Maintain accurate and up-to-date records of service user interactions, assessments, and care plans.
Requirements:
  1. CSW/CQSW/DipSW/BA/MA in Social Work
  2. Proven post-qualifying experience in working with Adult services
  3. Current Social Worker England registration
  4. Current Enhanced DBS Disclosure
  5. Eligibility to work in the UK without work permit or visa restrictions
  6. Commitment to the safe care and well-being of vulnerable people
